The thesis
Pluvicto label expansion proves the radioligand economics work
Pluvicto booked $642M in Q1 2026, up roughly 70% in constant currency on the pre-taxane mCRPC label (FDA expansion March 2025), putting it on a multi-billion annual run-rate and proving targeted Lu-177 therapy can move from late-line salvage to earlier-line use. Novartis is building plants (Indianapolis, Italy, Spain) because it judges demand to be supply-limited, not demand-limited. Every solid-tumour antigen with a small-molecule ligand becomes a radiopharma target template.
BMS/Lilly/AZ M&A locked in pharma-scale radiopharma platforms
BMS/RayzeBio $4.1B (closed Mar 2024), Lilly/Point $1.4B (closed Dec 2023), AZ/Fusion up to $2.4B incl CVR (closed Jun 2024) – three of the top oncology pharma companies bought radiopharma platforms, with Novartis already in. Roche, Pfizer and Merck are the remaining buyers; Telix, Lantheus, Curium and ITM are the remaining independents. Lilly shelving POINT PNT2002 after SPLASH shows the assets, not just the modality, still carry binary risk.
State of the art (2026)
Radiopharma is now a pharma-scale oncology category. Novartis Pluvicto (Lu-177-PSMA) booked $642M in Q1 2026, up roughly 70% in constant currency on the pre-taxane mCRPC label, putting it on a multi-billion run-rate and validating the move from salvage to earlier-line use. Lutathera anchors the neuroendocrine franchise. Big pharma has bought its way in - BMS/RayzeBio ($4.1B), AZ/Fusion (~$2B), Lilly/Point ($1.4B) - leaving Telix, Lantheus, Curium and ITM as the remaining independents. The next frontier is alpha emitters: RayzeBio RYZ101 (Ac-225) in the ACTION-1 NET trial and Telix TLX591, whose ProstACT GLOBAL Part 1 safety data read out at ASCO 2026. Ac-225 supply remains the structural bottleneck.
Ac-225 supply security is the moat
Alpha-emitter Ac-225 is produced in microgram quantities (~1.7 Ci/yr globally as of 2024). NorthStar RadioGenix Ac-225 2026 commercialisation, Niowave proton-linac output ramp, TerraPower Isotopes, DOE Isotope Program 200 MeV linacs at BNL/LANL/TRIUMF — companies with locked supply contracts (BMS/RayzeBio, AZ/Fusion, Lilly/Point) execute their pipelines; companies without one stall. Isotope supply is the new platform-economics dimension.
